Insulin syringes vs pen needles
A syringe draws a dose out of a vial. A pen needle screws onto a pre-filled pen that dials the dose for you. Which one you use is mostly decided by how your medicine is supplied, not by preference.
The short version
Insulin syringes and pen needles are not interchangeable. Syringes are for medicines supplied in vials and give you full control of the volume. Pen needles are for pre-filled pens, which dial the dose mechanically and use shorter, thinner needles. If your medicine comes in a vial, you need a syringe.
Side by side
| Syringe | Pen needle | |
|---|---|---|
| Works with | Vials | Pre-filled or cartridge pens |
| How the dose is set | You draw it up and read the barrel | Dial on the pen, usually 1 or half-unit steps |
| Usual needle | 29G to 31G, 6 to 12.7 mm | 31G to 34G, 4 to 8 mm |
| Mixing two insulins | Possible in one syringe if prescribed that way | Not possible |
| Unit cost of the consumable | Low | Low, but the pen itself costs more |
| Reading errors | Possible, see how to read the barrel | Rare, the dial shows a number |
When a syringe is the tool
- The medicine is supplied in a vial. This is the deciding factor almost every time.
- Two medicines are prescribed to be mixed in one injection.
- The dose is a volume rather than a dial setting, which is common outside diabetes.
- Cost matters and a pen is not funded for you.
When a pen needle is the tool
- The medicine comes as a pen. Pens do not accept syringes.
- Dexterity or eyesight make reading a barrel hard. A dial with a click is easier.
- Very small needles matter to you. Pen needles go down to 4 mm and 34G.
Do not mix them
Drawing insulin out of a pen cartridge with a syringe is a known way to get a wrong dose and is advised against by manufacturers. If you have a pen, use pen needles. If you have a vial, use a syringe sized to your dose. Our sizes guide covers that choice.
Common questions
- Can I use a pen needle on a syringe?
- No. Pen needles have a screw fitting for pens. Syringes come with a fixed needle attached. The two do not connect.
- Are pen needles more accurate than syringes?
- The pen dial removes the reading step, so dose errors are less common. A syringe read correctly is equally accurate; the difference is the chance of a misread, not the device.
- Which is cheaper, syringes or pen needles?
- Per unit, both consumables are cheap. A vial and syringes are usually the lower total cost. In Australia, people registered with the NDSS for insulin-treated diabetes can get both subsidised.
